Public school teacher certification form for the purchase of a personal computer and/or related devices as perCode of Virginia §2.2-2012

This Public School Teacher Certification Form is required to be completed by the Public School Teacher. Once the Form is completed, the Public School Teacher will receive an "Verification Code". The Public School Teacher will submit the verification code to the Computer Supplier for verification during the ordering process.
